Pre-surgical rehabilitation

Quadriceps muscle exercise

  • Sitting chair with backrest For the buttocks and back to lean back

Food and Drink

Eating foods that are full of nutrients and hygienic will help the healing process.

  • Should drink at least 8 glasses of water a day

  • Eat foods that are high in fiber. Because the use of pain medication after surgery can cause constipationใ Eating foods such as guava, papaya, apples, carrots, eggplants, morning glory and kale can help reduce constipation after surgery.

  • Eat foods that are high in iron, such as meat, animal offal, seafood, egg yolks, green leafy vegetables. Iron is an important mineral in the production of red blood cells replacing the lost blood from surgery.

  • Avoid alcohol for at least 48 hours before surgery.

  • Should stop smoking for at least 6 weeks

  • The day before surgery should eat foods that are easily digested and high in carbohydrates

Regular medication

Must inform the doctor if taking anti-platelet drugs or fibrinolytic drugs. 

Commonly used drugs that need to be stopped at least 7 days before surgery include

  • Aspirin (except aspirin 81 mg, do not stop)

  • Warfarin

  • Clopidogrel

  • Cilostazol (Pletal)

Other personal medication to be taken as usual. Except for the morning of the surgery, do not need to take medication regularly. Anesthesiologist will prescribe medicine for you.

House preparation and caregiver

  • During the first month after the operation, going up and down the stairs may be difficult. Since it still requires a walking aid. The bedroom should be on the ground floor. But if necessary to stay upstairs, it may have to be arranged on a bathroom floor so that you don't have to frequently climb the stairs.

  • You should arrange the house with a properly wide pathway and sufficient lighting to prevent falls.

  • Patients can walk inside the house. After returning from the hospital, He or she can walk to the bathroom, kitchen to warm food by him- or herself. It may need a caretaker to bring or prepared food during the first 2 weeks. Patients will walk better, farther and walk without using a walking stick during 2-4 weeks after surgery.

The day before surgery

Exercise of the hip muscles

  • Lying on its side on a flat surface

  • Count for 10 seconds

  • The day that must come to stay in the hospital

    • Regular service patients must go to the hospital 1 day before the operation day

    • Premium service patients can choose whether to stay 1 night before surgery or in the morning of the operation (arrive at premium ward before 7.00 am of the operation day)

  • No water, no food, at least 8 hours before the schedule of surgery

    • For regular service patients, no food or drink after midnight.

    • For premiums service patients who have surgical queues in the morning, no water and no food after midnight.

    • For premiums service patients who have surgery queues in the afternoon, no water and no food after 5.00 am of the surgery day.

  • Bring all the prescribed medication on the day of admission.

  • Private room

    • Premium service patients will get a private room without having to contact the reservation center.

    • Regular service patients can reserve a private room by contacting the private room reservation center before the surgery day. But the availability is not guaranteed depends on whether there is a private room available on the day of admission.

Surgery day

  • Morning 6.00 - 7.00 am.

    • Nurses will draw blood and provide IV fluid to reduce fatigue while fasting and waiting for surgery

  • Surgery

    • It takes approximately 3-4 hours in the operating room but the actual duration of the surgery is 2-2.5 hours.

    • Most patients will receive pain relief. With the spinal block since it is the safest way for patients.

    • Patients will be inserted urinary catheters in the operating room. After the anesthesiologist has finished spinal anesthesia so that it will not hurt when catheterization.

  • After surgery

    • After surgery, the patient should lift the leg higher than the chest level and apply a cold compress to the surgery area.

    • Patients can bend the knee immediately after surgery and can sit up on the bed to have dinner.

    • Recommend that patients drink enough water by looking at the color of urine coming out through the urine catheter If the urine is pale yellow, it means drinking enough water. If the urine is dark, it means that you have to drink more water.

After the surgery, day 1

  • Morning 6.00 - 7.00 hrs.

    • The nurse will remove the IV fluid and urine catheters to allow the patient to sit up and to walk more conveniently.

  • Late morning

    • The doctor will begin practicing knee range of motion exercise and teach you how to walk with a walker in case the patient is ready to walk in the morning.

  • Afternoon

    • Patients will visit the physical therapy department for gait training.

  • Recommendations

    • Patients should practice straightening their knees. Bend the knee as much as possible. Practice 1 set per hour, take 10-15 minutes, then rest and raise your legs high to prevent leg swelling.

    • Early knee flexion training may be painful at the surgical wound. After practicing often The pain will decrease respectively.

    • Practice walking into the bathroom with a walker and having close relatives at all times, should not walk without anyone to take care of. If the relatives are not there, they must notify the nurse to help. To prevent falls

    • Time to rest, patients want to rest More than sitting on a leg To prevent leg swelling

    • Drink enough water

After the surgery, day 2

  • Today will be the first day that patients will no longer receive injectable painkillers.

    • The doctor will check the wound before going home and change it to a waterproof dressing. So that patients can take a shower but not a bath.

    • The care will be the same as the 1st day after surgery.

Returning Home

  • 90% of patients will return home on the 3rd day after surgery and about 10% will return home on the second day after the surgery.

    • Before returning home, patients must bend their knees at least 90 degrees, able to walk to the bathroom with a walker by themselves and do not need injectable painkillers.

    • Patients who have not received a wound change will get it to change today before returning home. Some patients have doubts as to why the wound is not checked every day. In the event that the wound is dry well, Dr. Khanin's wound care approach is to minimize dressing change in order to reduce contamination. Bringing germs from outside to the wound but in the case that the wound has bleeding, the wound will be changed more often.

    • After replacing the wound with waterproofing materials, patients can shower. Pat the sheet dry. Patients will be given a spare of dressing material back home.

    • The patient will receive an appointment card to check the wound and movement of the knee joint 2 weeks after surgery
